Purpose
& Philosophy
In today's fast-paced
world, families' duties to take care of the elderly
can sometimes become a difficult task, especially
when those loved ones suffer from any type of
dementia. The purpose of our service is to provide
the best possible elderly care in a home-like
and secured environment.
The basis of our philosophy of care is a non-judgmental
atmosphere. Patience, understanding, compassion,
and professional knowledge are all fundamental
requirements for every one on our team of care
providers. |
Pre-Admission
Assessment
A pre-admission
assessment is conducted to ensure that a potential
resident would benefit from our program. The evaluation
process starts with an interview of family members
when they come to our care center for a potential
admission.
Care needs, social needs, dietary needs, preferred
activities: - everything is recorded and transferred
to our admission team comprised of the Administrator,
Director of Resident Services, Director of Medical
Services, Dietician and Activity Director. If
there is a need, our Clinical Psychologist or
Administrator may visit the resident at his/her
home or other location specified by the family.
The admission team uses the pre-admission assessment
as a starting point for developing a personalized
care program. |

Admission
Recognizing that
the placement can be a difficult decision for
both the resident and family, every effort is
made to assure a successful transition. Families
are encouraged to complete all admission paperwork
prior to the admission date. Furthermore, we suggest
that whenever possible, the admission to occur
in the late morning, just before lunchtime. This
allows the resident to spend time with the family
before eating lunch in the more social atmosphere
or in our private dining room. All staff members
are informed of the resident's history, hobbies
and personal characteristics, as provided by the
family. Family members are reminded that they
are always welcome to come or to call their loved
one on the telephone to help make the transition
more smooth and comfortable.
Our monthly support groups and our "open door"
policy give families an opportunity to discuss
their thoughts and feelings both with our staff
and other people who are in similar situations.
We have found that the peer support provided by
other families can be extremely helpful with the
admission process. A Few family members of our
previous residents voluntarily visit our care
centers specifically to help new residents and
families with the adjustment process.
After Admission
Residents are monitored
closely once they have been admitted to the care
program. On-staff licensed nurses head the ongoing
medical assessment. Information is compiled using
the physician's report, staff reports, the pre-placement
appraisal, and continuing information provided
by the family. RNs, LVNs, CNAs and care providers
carry out implementation of any recommendations
from such assessment. The administrator carries
out the social assessment based on input from
the family, a personal interview with the resident,
and observing the resident within the care center
environment. |

Activity
Program
The activity program
is designed to meet the physical, social and psychological
needs of each resident. Continuous activities
are offered seven (7) days a week from 9:30 A.M.
to 7:30 P.M. Our activity program is based on
the specific needs of our residents, and as such,
is continuously evolving. From large group activities
to one-on-one interaction; from participation
to observation, from physical activity to verbalization,
- there is something for everyone. Appropriate
activities include, exercise, music, reminiscence,
current events, dancing, therapeutic games, painting
and crafts, Community College Adult Education
classes, theme parties with professional entertainers,
religious services and neighborhood volunteers.

Staff
The Gardens care
centers are family-owned and managed by Isaac
and Stella Neches. They have been providing care
to the elderly of Orange County since 1983. Their
years of experience and dedicated caring has brought
them numerous honors and awards for the inspired
service they provide to those with age-related
conditions and their families. The staff at each
care center includes a licensed Administrator,
RNs, LVNs, CNAs, trained care providers and certified
Activity directors. Our staff's relationship with
the residents is based on respect, care, knowledge,
understanding, and patience. |
Physical
Environment
Our care centers
are located throughout Orange County. Please

Each of our facilities is located in an attractive
residential neighborhood. The 36 elegantly decorated
rooms of each care center are divided into wings
which all converge upon a bright and spacious
dining room. The recreation rooms are centrally
located around the dining room to provide a lively,
social and home-like atmosphere throughout the
care center. Sunny patios, beautiful gardens,
and walking paths are all located within the secured
grounds of each care center.
Our facilities are all Residential Care Facilities
for the Elderly (RCFE) and are therefore unable
to participate in the MediCal program. |

Changes
in Condition
The care providers
and nursing staff assess each resident continuously.
Any changes in the resident's condition are reported
to the attending physician. Any new physician
orders are implemented immediately and the resident's
family is informed.
